Percutaneous Endoscopic Laser Annuloplasty for an LPGA Golfer with Discogenic Low Back Pain due to the Annular Tear Syndrome

Abstract
The authors report a new percutaneous endoscopic laser annuloplasty technique to treat discogenic low back pain for a pro-LPGA golfer. A 31-year-old female patient presented with a 15-year history of chronic lower back pain, which was aggravated 3 years prior to admission. Due to worsening of her lower back pain, she was unable to play golf because of the effect on her golf swing. She was suffering from limitation of flexibility. Annulo-discography showed a leak of contrast from an annular tear to the peri-annular regions. An axial T2-weighted magnetic resonance image showed a high intensity zone with a black disc. Annuloplasty using a laser probe was carried out, starting laterally and moving medially across the midline to the opposite side. After surgery the patient reported subjective improvement in back pain. Currently, she is actively performing as a player and achieved a high rank in the LPGA championship. Percutaneous endoscopic laser annuloplasty using an endoscopic Holmium: Yttrium-Aluminum-Garnet laser for the treatment of discogenic low back pain is an effective and safe surgical option for a golfer.
